    No high-risk work without an approved, signed permit

    The permit goes through its authorization chain in order: supervisor, operations, and safety. Each step requires its role permission, and nobody approves twice in the same revision. Only once approved and signed is it activated within its work window.

    What changes: Work starts with a valid permit, not with a pending signature.

    Only qualified people go in

    Each permit type declares which qualifications it requires: work at height, confined space, rigger. When the permit is built, Safety shows who is fit and blocks anyone whose qualification or induction has expired.

    Contractors with their paperwork up to date

    Every contractor company carries its status, its approval, and its document compliance percentage. Each permit type defines which documents it requires and what happens when one is missing: block, warn, or allow an authorized exception.

    A critical finding stops the work and closes with verification

    Inspections run on versioned templates. If a critical finding is linked to an active permit, the permit is suspended. The finding is escalated to a corrective action with an owner, a due date, and subtasks, which only closes with evidence and a different verifier when required.

    What changes: What the round finds gets fixed, and someone checks it.

    Incidents reported on time and followed up

    Anyone can report from a QR code on site, without an account. The report comes in as unverified and the safety team decides whether to turn it into an incident. If an incident is still waiting for triage, Safety raises an alert based on its severity. Closing a high or critical one without a closed corrective action requires a justification.

    What changes: You hear about the near miss, not just the accident.
